As mentioned on your first thread, I have now submitted this thread to our developers so they can investigate further. If there are updates, you should know it via this thread.
Please send the fillable PDF form to firstname.lastname@example.org so we can include it in the ticket and so our developers can compare the output form and the PDF form. This way, we can also check what are the missing fields or incomplete.

its now the 25th Welvin & as requested I sent the fillable pdf form to support
I am trying import process again and hoping that it works this time around

a whole one minute and I have received the notification that the imported form is in the my form list - and yes i can see it and it has the correct title - with pdf symbol and the download pdf option
here's hoping it opens
