Every once in a while the forms lose the Google Spreadsheet integration

In addition, every once in a while the forms lose the integration altogether and then have to be reconnected. The notion of the integration is great, but it seems quirky at best

Apologies for the inconvenience. Upon checking your account, it seems that the forms are shared to you.

Could you share with us the forms that gets the problem with the Google Spreadsheet? Also, if you make changes to your form and the Spreadsheet, this may cause problems on how the data structure would match the spreadsheet which would break or lose the data. It would be better to re-integrate the forms. 

Could you tell us how often this happens in your end?
