this is not working at all for my pdf, it changes proportions on my header image, has duplicate information without the ability to fill out the field, overlaps text and fillable fields. The file is a complete mess...

    Thank you for contacting us.

I was able to reproduced the issues on your jotform http://www.jotform.com/50285819778168 when I test and created a Fillable PDF Form version.

The header field image has a distance gap from the top, which was probably due to the Form Tabs widget on the form.

I suggest you relocate the Form Tabs and Progress Bar widget at the bottom of the form when in the Form Builder so that the generated PDF Form will not add the space/gap for them.

Some fields and widgets were overlapping on the PDF form, so they become unusable on it.

Please take note that some of the widgets are still not available for used on the Fillable PDF Forms.

I recommend reviewing the user guide

for alternative option you can do on the form also to make it more usable when converted to PDF Form.

Unfortunately, this is not a bug, but just a feature that is not yet available. For the meantime, you can lessen the complexity by doing some re-design on the form http://www.jotform.com/50285819778168  so that it will work when converted to Fillable PDF version.

Please let us know if you need further assistance.

The thing is your form has too many widgets and there is no way we can import widgets to PDF forms. This also causes bad layout in your form. Thanks for your feedback.
