Add google map to the form with default location

ola , tem um templete que existe um mapa . Como faço pra deixar esse mapa focado no meu endereço , não em Hollywood ?

You have to set it on Google Maps first before you take the map embed code. https://www.google.com.ph/maps?source=tldsi&hl=en

Or you can use this widget and define your location. This is pretty easy than manually embedding the codes. http://widgets.jotform.com/widget/show_map_location.
