Submit error: Opps OAuth2 token error

We are getting this error on form 40936894873977. Please see attachment. 

Please let us know how to fix it.

I just cloned the form and appears to be working. However, I need to know what's causing this out of the blue so I can protect our other forms from going corrupt.

This will happen if you move your Google Drive folder or Spreadsheet after integrating.

I see that you have already sent the form into your trash folder, but if you restore it and recreate the integrations that you had - without altering the files and folders created everything should work properly.
