Form is not displaying all the content on smart phones

Our dog adoption application form is not displaying correctly on smart phones. It displays fine on a computer. I had to remove it from our website because no one could use it. The problem was in the display. Even though I set the form's size to be narrow it still didn't display correctly, portions of the text was cut off. Also, the next buttons weren't visible at all on the smart phone.

Is there some type of setting I need to use to enable the form to be displayed on other devices such as smart phone or ipad?

I tested the standalone version of the form and it displayed correctly on Android and on Iphone/Ipad:

Is the form only presenting problems on the embed version or also on the standalone version?

Which devices/OS are presenting troubles?
