How do i create a 4 column table where only one value is entered?

    How do i create a four column table where the end user has one selectable value?

    the other rows of the table is static information.

    Please see he form as developed in jot.form and the associated upload.

    You can use Radio Buttons instead of Check Boxes.  Use the following code:

    <input name="Shift" type="radio" value="Day" /> Day Shift <br />
    <input name="Shift" type="radio" value="Evening" /> Evening Shift

    instead of
    <input name="Day Shift" type="checkbox" />Day Shift<br />

    <input name="Evening Shift" type="checkbox" />Evening Shift</td>

    I changed all to radio buttons but it appears unless I use your application to create the grid, the user selected data is not captured.
    The html code produced a table as needed but the html was input using the Power Tools Free Text (HTML) tool you provide.
    It seems as though each row in the table should store and report a unique value selected for each row. As it is now, the entire tab does not produce any data when testing.
    How can this be resolved?
    If your application has a tool that allowed for developing this grid, I assume the data would be stored and reported.
    Please advise.

    When you need such a custom form structure, the best solution is to create a working form on Jotform with just the questions. Then copy the full form source code to your web site, and then make it look like that using a full HTML editor such as DreamWeaver. 

    Here is how you can get full source code. Open your form on the Form Builder:

    1. Open "Setup & Embed" tab on toolbar,

    2. Click on "Embed Form"

    3. Click on "Source" and copy the HTML code provided to your web site..
