Why does my form not load properly on my website?

There is 3 days ago that all forms i create are desactivated. i tried to create a new one and it doesn't load too.

Are my account disable?

Your account is not disabled and I am able to see your jotform on your website without any issues.

Now if you are still not able to see it I would presume that it might be caused by some conflicting code on your website (I did not see any shown), in which case I would suggest to try and embed the jotform with iframe embed code instead of js embed code you are using now.
