How do I set up radio button or "check box" questions so people can choose more than one option?

    I looked under settings and can't seem to find that anywhere.

    Hi judymac713,

    I think I can be of help to you.

    Radio buttons are designed to accept only a single selection. You can use checkbox instead for multiple selections.

     

    You can set up the checkbox for your multiple selections just like the way you did in your Age of Client question.

    Yes, as mentioned by chanesteves the radio buttons are set to only allow single option to be selected. This is a built in function in browsers and can not be changed, but - as mentioned the checkboxes would work for you and allow you multiple options to be selected without any alterations needed.
