I had the same issue and corrected the form by making it responsive, which adjusted the image banner and other standard text fields. However, the fields that are from Quick Tools such as, Full Name, Address, Phone, and Email are slightly over sized. How can this be corrected?

Upon checking your form, it seems the mobile responsive options were not enabled. I cloned your form, edited it in my end and add custom CSS code to make it fit to a mobile device. You can check the clone form here: http://form.jotformpro.com/form/50924994603967?.
Here's the guide on how to make your form mobile responsive: http://www.jotform.com/help/311-How-to-make-forms-Mobile-Responsive.
To clone my form and see the settings and use it, you can follow this guide: http://www.jotform.com/help/42-How-to-Clone-an-Existing-Form-from-a-URL.
I hope this helps.

I applied the solution to another form, but the Quick Fields are still over sized. I checked "Responsive in both Preferences and Designer. What am I missing?
