I have a form on a DNN web page that has worked for quite some time but has recently stopped working. By stopped, I mean that the captcha image loading icon just keeps spinning but never loads.

I'm sorry to hear that, I was able to see the problem when I visited the embedded form to your website, but not when visiting the direct URL of the form.
In your website, the captcha just spin and never loads the codes. You will have to click "refresh" icon to show it.
In your form's direct URL (http://www.jotformpro.com/form/20453317289), the captcha is working fine:
I think the problem is to the way how you embed the form, please try re-embedding it using our iFrame method: https://www.jotform.com/help/148-Getting-the-Form-iFrame-Code.

I'm sorry, I would like to highlight that the iFrame codes you are using is not from Jotform. It's a custom one. I have now submitted this thread to our developers, but can you try using our own iFrame codes? Just to see if the issue persists even when using our iFrame codes so I can also add it to the report.

I think that you should get the iFrame code rendered by our builder and maybe removing the <script> portion of the code should help.
Another solution would be to replace the current DOCTYPE declaration in your page form with this one:
<!DOCTYPE HTML PUBLIC "-//W3C//DTD HTML 4.01//EN" "http://www.w3.org/TR/html4/strict.dtd">
Anyhow, getting the iFrame code as rendered by our builder should fully solve the issue.
I was initially able to reproduced the incomplete loading Captcha using FF browser when I test the form on your website.
However after I have updated my FF browser to the latest version, it has now work properly and the Captcha value immediately load.
Please try also checking the browser if it is up to date or possibly have some add-ons/plugins that could be blocking the Captcha field.

I understand your point and I agree also in part to the Captcha field issue.
How about if you just enabled the Re-Captcha option of the Captcha field.
This option does not have the loading issue on the form when using different browsers.
Just to let you know, I also cannot re-open or submit yet this issue again to our higher team support as there is a viable solution available already to resolve it on the end-user side.

when you select the Captcha field it has a Re-Captcha property that you can set to ON (you can see it on the toolbar on top)
