Imported PDF form issue still a problem...

    Still have a problem.  I can’t make any sense out of the submitted data.  See what I mean below:

    Can you please provide us ID of the form so that we can take a closer look? I could not find that form under Email address that you used to post this thread. I have tested one of your forms and print button is giving printable form same as online version. 

    If you want that Header and text is shown inside of PDF submission you need to turn on option Show Header and Texts inside of Submission PDF.

    The form IP is 50.140.68.78

    And that option for show header and text isn't an option shown for me to check off.

    On IP 50.140.68.78 provided three accounts exist. On two accounts there are no forms and on one account there are 22 forms. Screenshots you provided are not matching with forms that I located on this account with forms. 

    Every form are located on some domain and has ID like sufix. E.G.

    http://www.jotformpro.com/form/50816187588973 

    Number marked with yellow is representing ID of form. 

    Can you please provide us exact ID of form which is causing you a problem?

    I'm on the account with 22 forms on it.  I have this same problem with several of my forms.  They were all forms that JotForms converted for me.  When I log into my account (the one with 22 forms in it), it says I'm guest user_5048492...  Seems like my account is a bit messed up...

    And that form ID is 50845174319963

    I have checked mentioned form and it looks like this. 

    Considering that your form look like this it is normal that your PDF looks like on picture you provided considering that you do not have labels on your check boxes.

    If you have changed your form, you can change them back to previous state by using Form Revision History.

    If you imported your form from Adobe Central and your forms are not looking now as expected please create separate thread on forum for problem you have.
