How do I transfer the existing form from another account to my new account

Previously I had access to jot forms with userid dlester1 assigned to email cttt@rafiusa.com. I created a form called the J E Dunn Application.

The primary question is: "How do I transfer the J E Dunn form that was created under the dlester1 userid to my new account?"

Can you please confirm that this is the form http://www.jotformpro.com/form/43485055014955

This form currently belongs to or owned by the jotform account/username dlester1

 Your new usenrame is dlester50  

We require the form owner/account to be always the one to initiate the request to transfer/move forms and submission data to another account.

This is for the purpose of security and confidentiality of the accounts.

If you can communicate with the account dlester1, perhaps you can request from them to send to us a request to transfer/move the mentioned form to your new account. 

Or you can also just request from them to grant you permission to clone/copy the form
