Please give details of how I can remove it from my device.after clicking on ok twice it disappears only to return again as soon as I move to another item.really haven't got time to mess with this.how dare you!

I am not sure if I have understood your issue correctly.
Do you mean to say that when you load your form iPad browser, you see "Jotform success"?
What is your JotForm account username or the email address you have used in your account profile? I am unable to find any details with your email address Etrl0029@gmail.com and hence unable to check any of your forms.
