Since you are not logged in to your account already and it seems you do not have access to the JotForm account as well, we need to verify your credit card information. Can you please answer the following :
1. Address used in the payment method.
2. Phone number
3. Last 4 digit of the credit card which is used for subscription payment.
4. Date of subscription renewal payment.
