Assigning form with indication on which form has been completed or not

    I am investigating methods of client forms management. We are opening an addiction recovery program that will include residential and outpatient services. We are expecting our volumes to include 30-70 clients in any given month with a unique patient count of 150-200 per year. We have approximately 20 forms that may be required of each patient. We need to track the completion of each of the forms by patient. One person at our company would be responsible for managing this work

    Ideally, each patient would be able to view a list of forms that have been assigned to them with the list indicating which forms have been completed. The patient should only be able to see their forms. The patient should not be able to see the names or information regarding other patients in the program.

    Is this possible with your application?

    What would the estimated licensing costs be?

    Thank you for reaching out to Jotform Support. If your form will collect patient information and health data then you can enable HIPAA Compliance on your account. HIPAA Compliance is available on our Gold plan. Please check our pricing page for your reference.

    HIPAA Forms encrypt the data right on your form and then transfer and store the data encrypted. The HIPAA form data is stored in our HIPAA-compliant servers which are separated from the Internet using multiple levels of protection and firewalls, and the encrypted data is stored in encrypted databases. In HIPAA-enabled accounts, only the account owner is able to view submissions made on their form.

    Does the application allow for the following -

    Hi Jerry,

    Thanks for reaching out to Jotform Support. It's possible to assign the form to them but they need to create their own Jotform account so they can view the forms. Assigned form will be visible on the Assign Form folder:

    However, it's not possible to automatically indicate which form has been completed or not but I think, they can based on the submission number. If it's 0, it means they have not completed the form yet. If says a number other than 0, then they already completed the form. Also, they can only view their own submissions.

    Here's how you can assign the form:

    1. On the Form Builder, click on Publish
    2. Select Assign Form
