I believe your account was suspended by our system due to your Card Authorization Form. When forms such as these are created, our system occasionally suspends the account for manual review. Since yours appears to be for legitimate purposes, I went ahead and reactivated your account.
Take care when collecting this type of information as it is stored in plain text in your account. Should your Jotform account be compromised, your data would be also.
