How can i add a check box mark(required to check) plus a text with a Hyperlink.

I'm was trying to do it by myself but i get so confused with html .

Your example is great but the "Check *" is way too far.

I want to use only this   .
The next thing is about opening a popup , and the last thing is how to add or inject this to my JotForm :) .

By the way you can add it :) this is the location where i want it to be.

Details:

The Form is : "Hotel Prince"

The text would be "*Estoy de acuerdo con los términos y condiciones"

Hi,

Based from the check box and the feature that you are looking for, it seems like it's the "Terms and Conditions" widget, you can add that to your form by following this guide: http://www.jotform.com/help/252-How-to-Add-a-Widget-to-your-Form 

You can add a different text and a hyperlink on it in the Wizards setting of that widget.

Unfortunately, this will not work as a pop-window but rather a separate tab when you click the link.

I already tried it but it doesn't work well with mobile, for instance if a customer is doing a reservation and he wants to see the terms and conditions it will take him to another site and erase all the stuff that he already filled in the charts, and a pop up would be the best choice for this problem.

It has many flaws :/, that's why i'm looking for something as i explained above.

It's important to have this "terms and conditions" for our company to avoid future problems with our costumers.

Hi,

Unfortunately, if you would really want a pop-up, you'll need to use a Javascript instead, you'll need to do a function and add it to the form's full source code.

In your full source code you can manually add a pop up modal using the jQuery dialog.

If you would like to design the check box almost the same with the widget, you can do that using the form designer. You'll need to use the default check box though.

In your form Designer Tool you'll see this option:
