502 Bad Gateway error shown when submitting form

    My Plane Fax Reports Request Form is freezing for customers and I'm receiving the error message:

    502 Bad Gateway nginx/1.2.3cNeed Help ASAP!

    Could you please let us know if you are still experiencing these issues?

    I took a look at your jotform, but it seems that there are no issues with it - there are few on your page, so I would suggest embedding the jotform using iframe embed code instead: How to get the jotform iFrame Code
