I need to send an email which contains a form in it (not a link to an external form) this form should send me back user selections within that form.
Any way to do this?

LOL, no this has nothing to do with email notifications.
I need users to be able to make selections WITHIN the email itself. in other words how can I embed the form IN THE EMAIL? while making sure that we get notified of their selections?
(kinda like an email survey)

Just to clarify also. In case you meant you want to embed the form in an email message , then that is not possible.
There are source scripts in the form that will be blocked by email clients/reader if you embed the script into the email message body. This makes it not possible to include script codes within an email message.
