Hello DennisH,
If I understand your issue correctly, it seems the full name is not being displayed in submission emails. Is that correct?
I did check your form's submission page, I found that the names are captured correctly but it is not being displayed in the submission emails.
Upon taking a closer look at your form, I found that you had a "Full Name" field earlier in your form. Later on you seems to have replaced it with three separate text boxes "First Name", "Middle Name" & "Last Name".
When you had full name field, you edited your email alert template. Please be noted that when you edit your email template, it becomes dirty. That means any changes you make in your form will not reflect in the submission emails. You will have to manually edit your email alert to add the correct field name.
Alternatively, you can delete the existing email alert of your form and add a new one. A new email alert will always display all the form field. Following guide should help you:

I have edited your email alert and all future submission email will display the name correctly. If you like, you can retain the email alert or add a new one.
