My form somehow stuck, not working on webpage.

Somehow stuck.

Sometimes works, sometimes not (after page refresh).

When not works http://prntscr.com/6rsb14 do not get red when is not fulfilled field, and without selecting (number 1..) shows thanks message (number 2..)

Can send contact without writing in the fields marked with *

Hi,

That's due to the conflicting scripts between the form and your page. Re-embedding the form using iframe embed code should resolve the problem.

Since the form loads in an inline frame container, it won't conflict with anything on your page. The form works similarly when you load its stand alone version.
