Hi there,
I have created one form which works perfectly fine on desktop and mobile however my new form is not working.

Looking at the subject above Sam I presume that the issue with the form above on the page you have mentioned above is related to mobile devices.
Could you please give us a bit more details as to what is the issue you are having and on which devices so that we can better assist you?

only the left part of the form is showing!

It is working fine on desktop but it will only show the left part of the form on my iPhone. I have used the iframe embed code like I have with my other forms and it is just not working.
i have also added the mobile responsive widget and it's still not working

Hello,
Your direct or stand alone form is mobile responsive but your webpage is not.
Please try to open your direct form and then re-size the width of the browser window, you will notice that the form is adjusting depending on the size of browser.
Now, do the same thing on your webpage: http://mxtimingtestsite.weebly.com/taupo-motorcycle-club.html. You'll notice that on the desktop even if you re-size the browser window width, it is not adjusting automatically. This issue is happening because your webpage is not responsive at all.
Since you're using Weebly then I would recommend you choosing a responsive template.
