Why is my form disabled?

Or if a form was made to steal user login credentials. All these activities are considered as Phishing and any account along with the guilty forms will be suspended immediately 

I checked your form and it has a "Facebook" logo and asking for the account email and password. Your account will remain suspended unless you delete that form and promise not to create another form that will violate our terms.
