Form Cover - Causing CSS errors

    Hey Guys!

    When I add a form cover with this header graphic below, it is throwing off some of the conditional fields to not operate properly.  If I insert the image, it doesn't give the same format errors - but I would like this to go across the form as a form cover instead.

    Here is the header graphic:

    Also, in trying to fix this, I have thrown off two dropdown fields that need to be properly centered in the form:
    #input_19, and #input_22

    Hello,

    Can you also described to us what were the conditional statement being disrupted by the Form Cover?

    I was checking your jotform http://www.jotform.com/51015965154958 but I currently could not immediately understand which conditions were not working at this time due to the existing Form Cover on the form.

    Please described to us with more details and we will check further.
