Why has my form changed?

    I have a quote submission form on my site and it has been working great until Otcober 23 when it stopped showing all the data (address) on my form when submitted. I have not done any updates to my site or changed any of the forms in jotform. your assistance is appreciated.

    Where are you not seeing the address data? I ran a test submission on a clone of your form and all of the submitted data showed up in both the emailed notification and submission records.

    If it's just the notification which exhibits the problem, try deleting it and replacing it with a new one but as I stated, the current one performs okay.
