We received one submission and I viewed the submission.  When I tried to look on the report, the form is no longer in file.  In the website it says deleted by owner.  What happen?

I checked your jotform account and found you have this http://www.jotform.us/form/50376335380151 Gift Membership Form for Greenburgh if that is the one you might be referring to possibly which is Active.

If you are still seeing a "deleted by owner" for your website form then I would recommend checking the embedded form to make sure the Form ID's match which is the string of numbers at the end of the form url and you should also be able to find that in its embedded code as well.

Additionally, as a precautionary measure I also had checked your Trashcan and found you have a second form of the same name in there.

This leads me to believe that if you are still not seeing your form displayed on your site with "deleted by owner" then that is probably the one you had embedded onto your site which would be the reason why this is happening. Whereas the above form I found appears to be the original since it already has submissions on it and this one in your trashcan has none.

If you didn't do this or have a copy of it then perhaps someone who you gave access to your account did this possibly? If that's not the case then let us know so we can investigate it.
