when I sent out my form to check, it was already filled with responses from a previous check.

I checked your form and found out that the Autofill feature is enabled. It is the reason why your form is pre-filled with data that was not submitted.
You can disable the Autofill feature or clear your browser cache to avoid form being pre-filled with data.
