I'm sorry for asking so many questions.. and you're very good at replying FAST!!
I did everything you suggested.. I'm still having the form partially filled out when I upload it.. I can now clear the form but when you go in again.. the same info is still there (I sent a screen print in the last email)

The autofill feature is now disabled in the form so I think the data is now just a stored information in your browser through Autofill forms by Google Chrome, kindly check:
Clearing your browsers cache and cookies should help you remove the data permanently in the fields.
