Do you provide a Business Associate Agreement?

I am opening a telemental health practice and want to have my patients be able to submit their patient intake paperwork, background questionnaires, etc. electronically through JotForm. This information is protected by HIPAA and HITECH. Do you provide a Business Associate Agreement for JotForm subscribers?

Update (April 19, 2018) HIPAA is available for our Gold and Silver plans.

We do not.  We are not an officially HIPAA compliant business.  You would need to setup and use our form in a HIPAA compliant manner and self regulate the process.
