Your form is currently disabled that is why it is not showing on your site.
If you want to enable your form back, go to your Archive folder. Select the form you want to enable then click More button, then Enable.
If you want to find a user who submitted your form, please check it in your form submissions page https://www.jotform.com/help/269-How-to-view-Submissions. You can also forward them to your email there.
