I am not exactly sure of the lorder form ayout as yet.
Have attached a link to a website that has an order form something along the lines of what I need.
Can this be achieved with jot form?

The form you mentioned is very much possible to create in JotForm. You will make use of some radio buttons, text boxes, and a payment tool integration to build this form in JotForm. You must have a Paypal, Google Checkout, Authorize.net, or any other payment gateways supported by JotForm.

Are there any other forms of assistance you offer that I could use
(excluding the forums of course).
Do you happen to have a step by step process I could follow to create the
order page form similar to the page previously discussed?
Do you offer the service where JotForms build the form for a fee?
