There is probably something super simple I am missing here, but all of a sudden I can't view my form. I have allowed popups and that does not seem to be the problem now. Help?

I checked your form itself and in the form builder from your account and both appear to be working properly. Is the form not working even if you copy and paste the link to your browser? As far as I can tell it should be working:
Even if you do have pop-ups blocked, the form should still load when you paste the link.
If you are still having trouble loading your form, let us know a bit more detail about what happens when you try to load it and we will be happy to have another look.
