Wrong price calculated in total of items

I have a uniform order form template. The first item on it is reunion T-shirt. That price of the shirt is $20 or $22 depending on the size ordered. Everything appears to be correct but when I get to the bottom of the form, I am charged $45. I can find no reason for this to happen. Can you please take a look at it and advise me? Also items are set up with "cancel" as the default selection. A couple of them fail to select "cancel" as a default setting. What would cause that?  Thanks

The price issue is caused by Calculation Values within 'Men's Long Sleve' section. There is one extra 25.00 value. Please remove it to fix the issue.

The default selection might be affected by enabled form Autofill option, so you might want to disable it.
