I set up my JotForm to integrate with Constant Contact, however Jotform is not sending the form information to Constant Contact.
I do not have any trouble with the setup up; everything seems to be going through, but when I fill out a test form, the information does not appear in the Constant Contact list.
I've removed and reinstalled the integration several times, but that hasn't fixed the problem.

I'm sorry for the trouble caused to you.
Have you mapped your form fields with the Constant Contact fields successfully? I did check your form's integration and it does not seems to display any list in the dropdown? Please check the screenshot below:
When you integrate your form with constant contact, it should display the list in the dropdown and then it will ask you to map the fields in which the contact data will be saved. Please check the screenshot below:
I did clone your form and integrated it with my constant contact account. It seems to work as expected. The contact was successfully added to my constant contact account. Please check the screenshot below:
I would suggest you to please re-integrate again and ensure that the form fields are mapped with your constant contact fields.
