The url it gives for the form looks a little squirrely: https://secure.jotform.us/dianehayes/http://www.graduatesform so I'm not surprised that it isn't working. What should I do?
I checked your form and the custom URL is currently set to https://secure.jotform.us/dianehayes/http://www.graduatesform
Since this is very much so not a proper URL, it is no surprise it is not working. To create a custom URL, you need only add the additional text you would like to add to the URL, not the full URL:
Yours currently has the full URL in the field, rather than just additional text:
Removing the http://www. portion of your entry in the custom URL field should allow for the link to your form to function properly.
