Form no Submitting on Firefox

Yeah, my doctor insisted on jot form and my default browser is fire fox on a Mac and it doesn’t work at all. It won’t let me submit the form. It wants me to check every box over and over again forever for infinity and I can’t submit it and it’s disgusting and they said it works fine for them so it must be me so I hate jot form and if a doctor forces me to fill out a jot form then I will not use that Doctor

If a doctor tries to make me use Jott form, then I will fire that doctor and look for another one
