A previous employee created templates using Jot Forms and they are published through you. I work for Great Expressions, and none of the current employees not how to create, edit, or administer this application. Please give me a call @ (248) 203-1100 ext 71197.

We/JotForm does not do phone support and also jotform is a very easy tool, you join, make forms, and publish the forms to your website while our support team is here to help 24/7! Give a little more detail of what you want please.

As far as I understand, you do not have an access to mentioned JotForm account. If that is correct, you may try to contact your previous employee and ask him/her to change account associated email to yours and provide you with the password, then you will be able to log in and manage existing forms/templates.
