Here is what I want it to look like on my website:
But it does not appear - http://www.nobrokenhome.com/ProductDetails.html

What website host are you using?

I use Intuit.. Homestead.. I've never had problems with them before nor with using Jotform..

Its ok its most likely not embedded correctly or combining woth another code, could you please make a seperate HTML area for that code!

What do you mean a separate HTML area.. When you place HTML in Intuit's Sitebuilder it automatically offers a separate HTML box to place code in.. so I'm a little confused. Thanks

One thing you could try, however unlikely it is to work, is to get your JotForm script embed code, right to the very bottom of your webpages code, just before the closing </body> tag.

Thanks.. I thought about that as well.. I have a lot going on..including mp3 downloads, shopping carts, etc. I tried it on the home page and same thing.. and that page has little going on..I also tried to bring it to the front of the other layers..
I will definitely try to move it further down the page.. I am so stuck on this particular embed option.. I like the way it looks on the page..
Thanks let me see..

OMG it didn't work.. I am so disappointed.. so disappointed. I also tried to put the code in between the html header.. ugh..

When I checked your page, I did not find any proper feedback form script in it. A feedback script should look something like this:
buttonText: "Sample Contact with confirmation page and send post data",
width: 700,height: 500});
But the italicized-bold part of the sample script that I shared is something that's missing in your code. Can you please try re-embedding your code again and see if it works?

You can't see the script because I removed it yesterday after it was not working. I will put it in again so you can see it.. but I should inform everyone that I cannot see code because I use Intuit's sitebuilder. I can put it in however I cannot see how its placed on the page.. I will call them to ask how to see html while designing my page.
I will input the script again. Thank you.
