I checked your form and it looks like you have all three notifications set up properly. They should all send to the addresses you are attempting to send them to. The sender email for all of them is properly set to "email@example.com" and as far as I can tell, will all send upon completion of your form. If any of the specifically are not working, let us know which address is not receiving them and we will check to make sure they are going through.
In the future, if you would like to setup your notifications to go to multiple addresses, you can enter multiple in a single notification by separating them by a comma ",":
