Form is not responsive

My form is supposed to be responsive, however it is not rendering properly on mobile platforms. I have tried embedding the code, source code, and iframe. It is not my website or the CSS because I'm using two others of your forms and they render just perfectly. When the forms in question were first embedded, they worked properly, then simply stopped.

I cloned your form and found out that you forgot to check the option to make form responsive in the Form Designer.

Please go to Form Designer. Under the "Form Layout", please enable the "Make this Form Responsive". Just save the changes then close the Form Designer or preview the form.
