    How do I link Lightbox forms to an image hotspot

    I have a question that simply comes from being a novice. I have a single page website that is an image with hotspots. I want the hotspots when chosen to cause my forms to pop up in their Lightboxes and have their masks cover the parent webpage. I have taken the Lightbox embed code as seen below:

    I am using Dreamweaver and have made the hotspot ON CLICK to CALL JAVA SCRIPT and am assuming that would be the "http://www.jotform.com/min/g=feedback". My question is where does the rest of the code after that go. Do I simply put it anywhere in the header or does it have to be in a particular place. Perhaps you could provide me an example so I can look at how it was set up by someone who has it working.
