We've had a ticket open since late last week with David which he hasn't responded to. Kris at GLSEN is attempting to receive emails to firstname.lastname@example.org however these emails do not appear to be hitting the external spam filter to be sent to the Exchange server at GLSEN. David had sent over a screenshot that emails are going out and requested form information. We haven't heard back in a week after repeated attempts.
Please look further into this issue.

This is the thread, Jason, http://www.jotform.com/answers/558091-Not-receiving-emails-from-JotForm. I didn't see any replies from you or your colleague(s) after David posted his reply to the problem. This was posted using a guest account, meaning, you are not logged into your registered account.
Another thread here http://www.jotform.com/answers/557867-I-m-not-receiving-the-messages-to-verify-my-email-address#1. This was posted using the account and replied by my colleague Raul.
Anyway, I checked the given form and the recipient is email@example.com. Did you just change it? Also, the sender email is firstname.lastname@example.org. If you have set some filters in your email servers, you should include this sender to your whitelist. Your form email configuration is correct and should work, but if the current settings will give you some problem, we recommend setting up SMTP instead. We have two methods, you can either use Mandrill or use your own SMTP details.
