I checked for any forms associated with your email address and I was unable to find any. If you wouldn't mind letting us know the form you are having trouble with and if it is embedded to a webpage, the URL of the page that it is embedded to. This way we can test to see why the form will not complete submission.

This is my husbands form (Craig). He asked me to test. It worked on my PC and IPad but not on my IPhone6. Thank you, Diane

I tested the form in a simulator and I did not have any issues submitting the form. Each time I was able to complete the submission:
The settings I tried are IPhone 6 running IOS 8.3, the most updated version and the browser tested was Safari. Could you let us know if your setup is different? Also, if possible, try using a different web browser other than the one you are currently to see if that resolves the issue.
