I am sending participants a copy of the form using an email notification with a PDF-Link (see below screen shot). I would like to have our logo on this document, but can't figure out how to get it to work based on existing documentation. Any assistance would be appreciated.

You would need to enable the "Show Headers and Texts" option in the submissions screen
And use an image field to add the logo to your form.
The logo should be included on the PDF file.
