    Hi, I am looking to create a risk assesment form that can generate a printable document once completed, but more spacifically if say question one is; Do you have a current curetificate of compliance for your electrical instalation? if there is a "Yes" responce no further action, if "No" information is generated into the action plan at the end of the document. If Jot form is not able to do this would you have any idea where I may be able to find somthing that does? Many thanks for you time PG

    You can use conditions to change the Thank You page of the form. So, you can basically make a separate thank_you_certification_plan.html page on your site and redirect users to this page if the user chooses "No" option.

    Another possible solution might be to use Send Post Data feature to send the data used on the form to your Thank You php file, and then show contextual information depending on what answer was chosen. This is more flexible but it requires some very basic PHP programming knowledge.
