Tried to add multiple instances of the same form on the same page

Is there a way of using the same form multiple times on a website.

Right now it won't let me use same because of multiple instances of same form on one page.

I have placed a simple Contact Us form within products that are view able on the same website. But only works for first.

It should not be a problem to have the same form several times on the same page.
