I've tried many of the different embed options and pasted each code in the web page I am working in and no matter which frame I use (Full HTML, Filtered HTML, Disabled Rich Text, etc.), the form doesn't show up. I've tried saving and returning, refreshing, etc. In most cases, the code just disappears after I save.

From screenshot provided I see that your are using some editor where you want to embed your code of the form. Are you using some CMS or similar?
Problems seems to me like 'editor' does not render code like HTML, it is rendering like simple text.
Did you try to publish your site after putting code in 'editor'? How website looks like?

Correct me if I am wrong but I think that you are using Drupal CMS. If you are using Drupal CMS you can follow this guide: http://www.jotform.com/help/5-Adding-Form-to-Drupal to embed form to the Drupal page. Basically, all you need is to switch your editor the Full HTML editing view and enable rich-text and paste the form embed codes there.
Or, you can use Widgets such: https://www.drupal.org/project/embed_widgets.
