How can I populate hidden contact info fields in a survey form from InfusionSoft contact data so I know who is responding to the survey without asking them for contact info again? I set up the InfusionSoft API integration.
I want to initiate the Survey invitation from InfusionSoft, and embed the survey form on my server. When a user clicks the link in the E-mail to take the survey, I want to populate their survey form results with contact info I feed from their InfusionSoft contact record.

I mapped the form fields to InfusionSoft Fields, but I'm not sure how to pass the data?

Hi,
If I understood you correctly, you are creating a survey form from InfusionSoft, you'll use JotForm to collect contact info, then that info will be prepopulated to your InfusionSoft survey form? Is that right? Or you are using JotForm as the survey form and prepopulate it by fetching existing contact info from InfusionSoft?
Unfortunately, this seems to be a different product, I'm not familiar with how InfusionSoft forms works and how data can be retrieved from your existing contacts from InfusionSoft, what JotForm integration does to your form is that it sends the data to your CRM, build contacts, leads, etc... You might want to ask the InfusionSoft support team if this is possible.

Another possible solution is that the contact info will be sent to your MySQL database using PHP, then, from there, you can create a PHP script to send an email with the survey form having prepopulated values (you can prepopulate the values by fetching teh existing contact info from your database).

Hi Charlie - I got InfusionSoft URL Parameters info and have created eh following links in the Survey invite:

Can you please look into Infusionsoft Integration?

Connect to Infusionsoft via API. Map Jotform Fields to Infusionsoft Fields. And setup actions to be triggered upon form submission.
Can this resolve your problem?
