Hi, this is happening again with the submissions from Anissa Jones and Dimitria Harding on this form

I am able to replicate the issue with the two submissions below :

I have searched for the said files, unfortunately the said files could not be found. It may have inadvertently got lost while the files were being submitted. This happens when the file was being uploaded at the certain period where the internet connection of the person submitting the file was intermittent. As a result, causing the submission to prematurely complete the process.
What I can recommend to you is to contact the people who submitted the file and request it to be submitted again.
